Covid 19 Office Policy

Dear patients,

Dear Patients: As you know, Governor Greg Abbott has announced that he will lift the statewide masks mandate effective on Wednesday, March 10, 2021.
At the beginning of the pandemic, our office established COVID-19 procedures and protocols to ensure the safety of our patients and staff. We will continue to require our staff and patients to wear masks as recommended by the Centers for Disease Control (CDC) while in our office and building.
At Dr. Shimizu's dental office, we continue to focus on the wellbeing of our patients and staff.

When You Arrive We Will Ask You To:

• Remain in Your Vehicle
• Check-in from Your Car
• Sanitize Your Hands
• Complete Your Covid-19 questionnaire
• Get a Temperature Check
• Properly Wear a Mask Throughout Your Entire Visit
